Special Toll Free Dial-In Plan Request Form

The requestor of the Toll Free Dial-In conference call will be provided with the conference bridge phone number, access code, and the requestor's password code. The requestor of the conference call will need to provide the phone number and access code to other members of the call. The requestor will be charged $0.035 per minute per participant. A confirmation E-mail will be sent to the E-mail address provided by the requestor when the conference call is set up.

This conference call will be set up as an ongoing AT&T Toll Free Dial-In Conference Call and may be used over and over again on a 24/7 basis. You will not need to make another reservation. It can be used by others in your department, as applicable, but the department will be billed for conference call service in the name of the original person * making the conference call to the “Account Number to be Billed” listed below.
