Banner Enterprise Access Request Form Instructions
This page describes the process of requesting access to the Legacy (Mainframe), SQL Server Data Warehouse, and/or Banner Enterprise systems. If you have any questions about the access request form or process, please contact EAS at EAS.IS@ttu.edu. The steps to obtain system access are:
1. Complete the required Requestor information on the Banner Enterprise Access Request Form.
- Last Name, First Name, Middle Initial (MI): Please submit your last name, first name, and middle initial in this area.
- eRaider Username: Your eRaider identifier (e.g. rraider)
- Banner Oracle ID: You may not have a Banner Oracle ID (also called a “License Plate”) yet, so unless this is a request to Add Access, Remove Access, or Terminate Account, it is acceptable to leave this field blank.
- TechID: This is your new Banner identifier which begins with an “R”. If you do not know it, and unless this is a request to Add Access, Remove Access, or Terminate Account, it is acceptable to leave this field blank.
- E-mail: Please provide your primary TTU.EDU or TTUHSC.EDU e-mail account.
- Phone Number: Please provide your office contact number including extension, if applicable.
- Campus: Please check your campus of affiliation.
- Department/Org Name: Name of your department or organization.
- Former Department/Org Name: If this is a request to Add Access, Remove Access, or Terminate Account, please provide prior department information. For Create Account requests, it is acceptable to leave this field blank.
- Department/Org Code: Code of your department or organization.
- Former Department/Org Code: If this is a request to Add Access, Remove Access, or Terminate Account, please provide prior department code information. For Create Account requests, it is acceptable to leave this field blank.
- Requestor’s Signature: You may print and sign, or include a digital signature. Unsigned applications will be returned.
2. Ask your supervisor to complete the required Supervisor information.
- Select one (1) of the following:
- Create Account: This option is for new Banner Enterprise accounts. If your employee has never used any of the Banner Enterprise products, please select this option.
- Change Account Access: This option is for current Banner Enterprise accounts. If your employee needs additions or changes to his/her account access, please select this option.
- Terminate Account: This option is for current Banner Enterprise accounts. If your employee has left or is leaving your department or the institution completely, please select this option. Urgent requests can be e-mailed to EAS.IS@ttu.edu.
- Select the appropriate systems and auxiliary systems that your employee needs to perform their job duties.
- Finance (FI)
- Human Resources (HR)
- Student (ST)
- Supervisor’s Name: Please submit your full legal name in this area.
- Supervisor’s Title: Please provide your official title.
- Supervisor’s Signature: You may print and sign, or include a digital signature. A signature of departmental approval is required. That signature must be the applicant's supervisor who has signature authority for that department. Unsigned applications will be returned.
3. Submit the Banner Enterprise Access Request Form to the Enterprise Application Security Team.
- Access requests should be submitted at least 1 week before required training to ensure enough time to process the security request.
- Electronically: If you completed the form on your computer and included a digital signature, attach the form to an e-mail and send to EAS.IS@ttu.edu.
- Mail/Campus Mail: If you completed the form and printed to sign, please mail to EAS at MS 1095.
- Fax: If you completed the form and printed to sign, you can also fax to EAS at 806-742-1366.
4. Schedule and attend required training.
- Because of the sensitive nature of data housed within these systems, you are required to attend training before use of the systems is granted. If you do not know what training may be required, please contact EAS at EAS.IS@ttu.edu.
